CAE/CSSM RN Supervisor - Full Time, (Position Filled)

 

This full time position is responsible for overseeing the Clinical Assessment and Eligibility (CAE) program and Comprehensive Screening and Service Model (CSSM) program. This includes RN services within components of CSSM such as identifying eligible consumers or assisting consumers returning to Friendly Connections – Social Elderly Care Center.

 

Other responsibilities include, but not limited to:

 

  • Ensuring compliance with regulations and documentation requirements.
  • Providing program statistical reports and quality assurance.
  • Assisting in developing policies, procedures, and objectives.
  • Promote community awareness of our services.
  • Providing case consultation and reviewing case recording
  • Providing supervision to department members including staffing, orienting, training, appraising performance, setting performance goals, resolving employee relation issues, and building teamwork.  Sharing clinical expertise, acting as a resource person for staff in addressing special needs of our clients. 
  • Attending statewide EOEA or RN Supervisor meetings, along with interdisciplinary in-house meetings to discuss consumer issues.

 

This position requires a Registered Nurse Licensed in the state of New York, demonstrated leadership and supervisory skills, as well as organizational, documentation and computer skills.  Excellent interpersonal skills are necessary for appropriate interaction with consumers and their formal or informal supports, and in acting as a liaison with other area health care providers, home care professionals and regulatory representatives.

 

Care Manager - Full Time, (Position Filled)

 

The Care Manager provides client-centered assessments and then authorizes, coordinates and monitors services provided to elders to assist them in living independently. We are seeking an individual with a Bachelor’s degree in social work or related field.  Effective communication skills are necessary for interaction with clients, families and other supports.  Must be able to utilize computer for data entry, word processing, and research plus experience with elders and/or in a community service setting Is preferred.

 

Nutritionist – Part Time (10 Hours), (Position Filled)

 

Friendly Connections, Inc., meeting the needs of older adults in the heart of Ridgewood seeks a nutritionist/dietitian for our Elder Nutrition Program.  This is a dynamic 10 hour per week position offers the opportunity to assume a variety of roles.  We seek a candidate who enjoys communicating the benefits of good nutrition through written and oral communication as well as providing assistance with menu planning and reporting, nutrient analysis, and other tasks necessary to ensure that the meals we serve meet our high standards for quality, nutritional value and safety.

 

We are seeking an individual with a Nutrition or related Bachelor’s Degree. RD or RDE preferred but not required. Two or more years of related experience are preferred with the demonstrated capability to create and conduct nutrition education programs, utilize computerized nutrient analysis tools, translate knowledge of menu requirements into appropriate, palatable menus, and communicate well with diverse audiences. 

 

Bi-lingual Elderly Care Specialist - Part Time, Intern/Volunteer

 

We are seeking an individual with fluent in English and ability to understand; speak Cantonese, Tagalog or Spanish.  This position under the supervision of professional staff may be assigned to assist with special program/participant needs (i.e., recreation program, rehab/maintenance exercise program, and internal program committees).  Provides behavioral/emotional supervision or support to participants, such as reality orientation or reminders to follow through on specific care and diet restrictions.  Other duties may include assistance with meal preparation, including special diets and set up for meals at the center.  Responsible for keeping areas of elderly care clean and safe when participants are on site plus regularly reports on status and progress of participants to Lead Care Specialist and Center Director.
